The Role of ADOS-2 in Adult ASD Assessment
The Autism Diagnostic Observation Schedule β Second Edition (ADOS-2) is a widely recognised, standardised tool used in the assessment of Autism Spectrum Disorder across all age groups, including adults.
In adult assessments, the ADOS-2 is used to observe and evaluate key social, communication, and behavioural patterns that align with the DSM-5-TR diagnostic criteria for ASD. It provides a structured setting where the assessing clinician can directly observe behaviours that might not be easily captured through self-report questionnaires or interviews alone.
What the ADOS-2 Involves:
- A semi-structured, interactive session (usually 40β60 minutes)
- Activities designed to prompt communication, social interaction, and responses to various social scenarios
- Tasks and conversational prompts adapted to suit adult language levels and life experience
Why Itβs Important:
- Objective observation: It supplements self-report and history-taking with direct behavioural evidence.
- Supports differential diagnosis: Especially useful when other mental health conditions (e.g. social anxiety, ADHD, trauma) may present with overlapping features.
- Strengthens diagnostic confidence: Particularly helpful in cases where symptoms are subtle, masked, or camouflaged.
Who Conducts It:
The ADOS-2 is administered by trained clinicians (usually psychologists or neuropsychologists) who are certified in using the tool and experienced in assessing adults on the spectrum.
When Is It Used?
Not all adults undergoing ASD assessment will need the ADOS-2. It is most commonly used when:
- The clinical picture is unclear or borderline
- Additional observational data is required
- Masking has obscured traditional indicators of autism
- The assessment team seeks to corroborate findings from interviews and questionnaires
In our clinic, the decision to include the ADOS-2 is made after a comprehensive review of your history, presentation, and initial assessment data. If used, it forms a critical component of the overall diagnostic formulation.
